Fees & Insurance
A non-refundable $20 fee is due when you check in at Patient Services for your screening appointment. You are expected to pay the day of your appointment as the work is completed. Lab fees and other miscellaneous fees require full or partial payment before your treatment begins.
Â鶹´«Ã½ students, faculty, and staff are eligible for a discount if they present a valid, current Â鶹´«Ã½ ID at each appointment. Implants are excluded from the discount.
Payment
We accept several methods of payment: cash, debit, money order, Visa, Mastercard and American Express. We do not accept personal cheques.
While we do not have a payment plan, you can put money on your account toward future treatment. If your account is not paid in full after 90 days, you will be suspended from treatment.
After we receive your payment, you must pay a $25 administration fee before we will consider accepting you back as a patient. We reserve the right to send any account with an outstanding balance to a collection agency.
Insurance
Please keep your receipts for income tax and insurance purposes. Please note, we do not accept direct assignment of fees to insurance companies. You must pay for your treatment in full. We will complete an insurance form for you to submit for reimbursement when your treatment is complete.
