[full-time and part-time (50% or greater) employees for contracts of at least 8 months or greater]

Forms for new employees

  • Mandatory forms
    • (complete if applying 60 days after hire date)
  • Registered Retirement Savings Plan (RRSP)Ìý
    • Grant holders must provide eligible Grant-Paid employees the option to contribute to the RRSP. If opting into the RRSP, the grant holder mustÌýmatch the employee's selectedÌýcontributionÌýrate ofÌý3%, 4%, orÌý5%. If enrolling in the RRSP, there is a two-step process.
      • Step 1: Complete the (login required)
      • Step 2: Complete the Manulife RRSP Enrollment

  • Optional forms
      • This form is required if an employee wishes to enroll in optional benefits.
    • Long-Term Disability
      • is required if an employees wishes to apply for long-term disability 31 days after their hire date.
